Skilled Nursing Care

Skilled nursing is the core medical service in home health care. It’s delivered by licensed nurses (RNs/LPNs) under a physician’s direction and focuses on clinical oversight and treatment at home. 

Assessments:
Nurses perform ongoing health evaluations—checking vital signs, symptoms, and overall condition to detect complications early.

Monitoring:
Continuous observation of conditions like blood pressure, blood sugar, infections, or post-surgical healing to prevent hospital readmission.

Wound care:

  • Cleaning and dressing surgical wounds, ulcers, or injuries
  • Preventing infection and promoting healing
  • Using advanced treatments when needed


Medication Management and Administration

This goes far beyond just “giving meds.” It’s about ensuring safe, accurate, and effective medication use, especially for patients with complex regimens.

  • Administering medications (oral, injectable, or IV)
  • Creating medication schedules and organizing doses
  • Monitoring for side effects or adverse reactions
  • Educating patients and caregivers on proper use


IV Therapy and Injections

This is advanced medical care at home, often used when oral medications aren’t effective.

  • Starting and maintaining IV lines
  • Administering:
    • Antibiotics
    • Fluids (hydration)
    • Pain medications
    • Nutrition (TPN)
  • Monitoring for complications (infection, reactions)

IV therapy shows that home health care can provide hospital-level treatments in a home setting, especially for:

  • Serious infections
  • Chronic illness management
  • Recovery after hospitalization


Post-Surgical Care and Recovery Support

This service helps patients transition safely from hospital to home after surgery.

  • Monitoring surgical sites for infection or complications
  • Changing dressings and managing stitches/staples
  • Pain management
  • Mobility support and coordination with therapy
  • Educating patients on recovery steps and warning signs
